Wondrous Item (cloak), very rare (requires attunement)
This cloak is one of several artifacts recovered from the hidden sanctum of a cult devoted to the Archfiend of Love—Ishqara, the Thorned Temptress. Luxurious and silk-like, the cloak is pure white on the outside, marred by faded bloodstains that refuse to wash away. Its inner lining is a deep, carmine red, woven with countless fine, needle-like thorns that prick the flesh with even the slightest movement. Etched between the seams are intricate celestial runes, enchanted to twist the sensation of pain into euphoria, fortifying the wearer's mind against external influence. Yet this gift comes at a cost: the cloak curses its wearer with an insatiable craving for agony. Without pain, they find no peace—unable to rest, unable to dream.
While wearing this cloak, you have resistance to psychic damage and are immune to the charmed and frightened conditions if the effect that imposed the condition also dealt damage to you as part of the same action or instance. The cloak twists pain into pleasure, overwhelming your senses and guarding your mind against outside influence.
Thorn of Ecstasy. When you are hit by a melee or ranged weapon attack, grappled, or fall prone, the cloak's inner thorns dig into your flesh, dealing 1d4 piercing damage. This damage can't be reduced or avoided. You can add the most recent roll from this piercing damage to one attack roll, damage roll, ability check, or saving throw you make before the end of your next turn. If you take damage that includes this piercing damage and must make a concentration saving throw, you can use your reaction to succeed automatically if you roll 3 or higher on the piercing damage.
Curse — Craving for Agony. Once attuned, you are cursed. You cannot willingly end your attunement until the curse is broken with a remove curse spell or similar magic, and removing the cloak does not lift the curse. If removed, the cloak reappears on your body the next time you sleep or complete a long rest.
If you finish a long rest without having taken damage in the past 24 hours, you gain no benefit from that rest. To satisfy this requirement, you must take at least 5 + your character level in damage that reduces your hit points. Damage absorbed by temporary hit points, reduced by resistance, or immediately healed does not count—the pain must be endured.
Over time, wearers of the cloak may become dependent on the sensations it inflicts, seeking pain simply to find rest.
