It probably won't make much sense to you and you hate it, but the reason it's still Jim that we're seeing is because of his spirit being in Sam's body and Melinda sees spirits, so we're seeing him through Melinda's eyes. It wouldn't make sense for us to see the actual Sam guy's face when he crossed over and his spirit isn't in his own body anymore, it makes sense if you actually think about it that way. Sam as you noticed is only going to show up in reflections. This is a journey of Melinda and Jim getting to know each other and falling in love again, but for the second time.
