- This form is only for anyone who had Jay as an original team member and needs a replacement to fulfill the requirement of 3 team members (1man, 1 woman & 1 man or woman)
- If you pick a replacement for Jay, your new team member will get points as usual - but they won't get placement points if they finish 1st, 2nd or 3rd
- If you had Jay as a captain in ep18 - I'm sorry but you can't pick another captain for ep19
