RULES FOR THE FINAL:
- With this form you are picking a captain that you will have for THE WHOLE FINAL - you will have 1 and only one captain for the next 2 episodes
- If the captain you picked is eliminated, purged, medically dq'd or quits - you are out of luck (there's no picking a 2nd captain in the FINAL)
- If you lost either James, Michele or Olivia in the previous episode; you won't have double captain points in ep18 BUT WILL HAVE DOUBLE CAPTAIN POINTS IN EPISODE 19 (as long as you don't lose a team member in ep18)
- Reminder you can't pick the same captain 2 weeks in a row so whoever you chose last week is ineligible.
- Also none of your team members are eligible to be your captain.
- If you had James, Michele or Olivia as a team member, please choose a replacement player at the bottom. The bottom section is only applicable to the people who had James, Michele or Olivia as a team member; not a captain.
- And there is a place to pick a 2nd replacement and that is to be used only if you had any two of James, Michele or Olivia as a team member and need a 2nd replacement to build up your team to the 3 person total.
- If someone from your team is eliminated, purged and/or medically dq'd in ep18 - you will be able to pick a replacement player for ep19. BUT if a team member quits - you cannot pick a replacement player for ep19
